Job Description

Standard Live-in Care Provider Assistance (Included) $1500/mo + Free Room & Board

Shopping & Meal Preparation
Cleaning
Laundry
Assisted Daily Ambulation
Yard care/Snow removal
Reminders to take prescribed medication
Transportation
Daily Record Keeping

The Additional Assistance Activities Listed Below (Extra Costs)

Bathing/Showering assistance
Regular bathroom assistance
Changing Depends/Pads
Daily Dressing w Assisted transfers in and out of bed
Assistance with eating meals
Nighttime assistance
Full body transfers & Dressing Daily

*Additional medical needs may be provided by Home Health Agency and exceed the care requirements CP can provide

Family and Elderly Client Application Content to be shared with selected care provider couples

Minimum Qualifiers for Live-In Care

A. The ability of the client to recognize danger and get out of the house in the case of a fire

B. Mental Acuity that does not include an Alzheimer's diagnosis

C. Client able to toilet on their own

D. Available extra bedroom and separate bathroom

E. Family able to pay $1500/mo for Live-In Care and Geriatric Care Provider Case Management.

Client Profile Information the Live-in Care Provider(s) needs to make a confident decision on a 12-month contract:

Provide a short video interview of your Mom/Dad (Grandparent) advising young people about where to look for true joy and happiness in life.
Please share a 1-2 minute video of the front/back/Inside of Mom/Dad’s home and the 3-4 areas he/she spends the most time. Please include a video of the separate room and bathroom the care provider(s) will utilize.
Please share a 60-second video of what the live-in care providers can expect on the hardest days caring for Mom/Dad and who and how you handle those days now
Please share a 60-second video from the family member with Power-Of-Attorney, any specifics the care providers should know about you & your preferred communication style.
Provide a short video addressing your Mom/Dad’s mental & physical health (good and bad)
Upload 10-12 pics of Mom/Dad/Grandparent over the years to build an image collage
Build a 20-25 item word cloud with descriptive words uniquely describing Mom/Dad
Upload a written list of the 10 favorite meals and treats for Mom/Dad
Upload a written Do NOT Resuscitate DNR order signed by a Physician if this is the client’s wish
