You are the dedicated, professional Community Health Worker for the Wells County, Indiana area. As a Community Health Worker you will act as a frontline public health worker who is a trusted member of the community and/or has a remarkably close understanding of the culture of the community served. This trusting relationship enables the CHW to serve as a liaison between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Community health workers provide outreach, education, referrals and follow up, case management, advocacy, and home visiting services. Community Health Worker will be a member of a medical care team in accordance with NCQA – Patient Centered Medical Home standards.
This is a full-time position working Monday through Friday 8am - 5pm. Travel up to 60% will be required to other locations as necessary. Travel within Wells county including Ossian, Markle, Bluffton, and Warren locations.
